You can save your amount in a disciplined manner during the policy tenure and create a corpus for your child’s higher education, for buying a home, for your child’s marriage, or for any other need with your Maturity benefit amount and Survival benefit amount (as per your policy T&C).

To Claim your Maturity Benefit and Survival benefit, follow the below process.

Survival Benefit :

Survival benefit is given to the policy holder during the policy tenure. As per the terms and conditions of some products, a certain predetermined amount is paid to the insured after regular intervals. Survival benefit applies only in cases where the insured person is alive.”

We send advance intimation along with Survival Payout Form, prior to the Survival Benefit due date at customer’s registered address. 

To enable payouts on due date, it is advised to update your recent Contact Details, Bank Account Details and PAN Number. You can log on to MyAviva to update your details.

To avail Survival Benefit, you can download and fill the Survival Benefit Form and submit the same at any Aviva Branch or courier it to Aviva’s Head Office along with other documents mentioned in the form.

List of Mandatory documents :

  1. Copy of cancelled cheque with pre-printed name and account number or self-attested bank statement reflecting the last 3 months transactions.
  2. Copy of self-attested photo ID proof of policy holder.
  3. CRS/FATCA addendum to be filled only if tax jurisdiction is outside India.

Note:

  • Please be informed that as per “Know Your Customer (KYC)” guidelines, you are requested to share any of the below mentioned officially valid document as an ID proof.
    1. Passport
    2. Voter's Identity Card issued by Election Commission of India
    3. Driving License
    4. Job Card issued by NREGA duly signed by an officer of the State Government
    5. Letter issued by the Unique Identification Authority of India or National Population Register containing details of name, address and Aadhaar number.
  • Kindly submit only the masked Aadhaar card copy to us where only the last 4 digits of your Aadhaar number should be visible to us. Please do not share unmasked copy of your Aadhaar card
  • For NRE Account Type, kindly submit Pre-Printed cancelled Cheque of NRE Account and Self Attested Bank Statement/Passbook of NRE Account from which premiums are remitted. The statement should reflect all the premiums paid through NRE Account.

 

Maturity Benefit : 

Maturity Benefit refers to the benefit that arises on completion of policy term as per terms and conditions of the policy.

We send advance intimation along with Maturity Payout Form prior to the Maturity due date at customer’s registered address. 

To enable payouts on due date, it is advised to update your recent contact details, bank account details and PAN. You can log on to MyAviva to update your details.

Download and fill the Maturity Benefit Form along with other documents mentioned in the form and submit the same at any Aviva Branch or courier it to Aviva’s Head Office.

  • Maturity Payout Form duly filled & signed. Click here on the link to download the form.
  • Self Attested copy of ID proof.
  • A Cancelled Cheque with pre-printed name or copy of bank statement /passbook of last 3 months duly attested by the bank.
  • Address proof (in case customer is Non-Resident or change in address)
  • CRS/FATCA addendum to be filled only if tax jurisdiction is outside India or click here on below link to download the form.

An annuity is a type of pension product which provides an opportunity for long-term investments. Most of these plans are available as Life Insurance policies from the best life insurance companies in India.

Scanned Documents required to be attached along with an email:     

  1. Duly filled Annuity Proposal Form (to be collected from nearest branch)     
  2. Copy of PAN Card 
  3. Customer ID & Address Proof
  4. Coloured passport size photograph
  5. PEP Declaration and Political Exposed Person questionnaire
  6. E-Account Declaration
  7. GST Addendum
  8. Form 15G or 15H ( 15H for Senior Citizen and 15G for below Senior Citizen)
  9. If Annuity Plan 7 (Aviva Annuity Plus - Life ( Joint Life Last survivor) opted by customer then 2 Annuity Proposal forms to be collected with above mentioned documents.
  10. NRI Questionnaire  (For NRI Customers).
